Allgemeine Geschäftsbedingungen für Doulas

This Doula Privacy Notice (“Doula Notice”) supplements Carrot Fertility, Inc.’s (“Carrot,” “we,” or “us”) general Privacy Notice and applies specifically to personal information we collect from and about individuals who apply to, or participate in, Carrot’s doula directory program (the “Doula Program”). This Doula Notice should be read together with Carrot’s Privacy Notice and Terms & Conditions for Doulas. To the extent this Doula Notice conflicts with the general Privacy Notice with respect to doulas, this Doula Notice controls.

INFORMATION WE COLLECT

When you apply to, or participate in, the Doula Program, we collect information you provide to us directly, including:

  • Your name, contact information, location, and professional background;
  • Certifications, training, and references submitted to establish your qualifications as a doula;
  • Information about your professional liability insurance policy, including the policy’s effective date and expiration date; and
  • Feedback and communications relating to services you provide to Carrot members.

HOW WE USE AND SHARE YOUR INFORMATION

We use the information described above to evaluate your application, confirm your qualifications, and administer your continued eligibility to provide services to Carrot members. If you are approved to participate in the Doula Program, we display certain information about you — such as your name, location, qualifications, and eligibility status — in Carrot’s Provider Finder so that members can identify and select you as a doula.

APPLICATION STATUS UPDATES SHARES WITH MEMBERS & CUSTOMERS

Because members rely on the Doula Program to determine whether the services they receive are eligible for reimbursement, we may share limited, factual updates about the status and outcome of your application, and about your continued eligibility, with (a) a member who requested or initiated your application, has selected you, or is receiving or arranging services from you, and (b) where needed to administer the Carrot benefit or respond to an inquiry or escalation, the Carrot customer (such as an employer or plan sponsor) through which that member receives the benefit. These updates may include whether an application has been received or is under review; whether it has been approved or not approved; the date your eligibility begins or ends; and whether documentation required for eligibility — such as an eligible certification or a current certificate of insurance — has been received, has not been received, or has expired. These updates describe the status of your application and the documentation Carrot has received; they are not evaluations, ratings, or characterizations of you or of the quality of your services. By submitting an application, you consent to these disclosures.

PROFESSIONAL LIABILITY INSURANCE EXPIRATION DATE

When required by the program based on the country where you provide services, maintaining professional liability insurance is a condition of your continued eligibility to participate in the Doula Program.

By applying to, or participating in, the Doula Program, you agree to provide Carrot with accurate information about your professional liability insurance policy, including its expiration date, and you consent to Carrot sharing that expiration date with members. Carrot will display this information in the Provider Finder and will also communicate it to a member when that member selects you as their doula, for example:

“[Doula name]’s professional liability policy is set to expire on XX/XX/XXXX. Please confirm with both Carrot and your doula that your doula eligibility will remain active for the entire period services are provided; otherwise, the service might not be covered by your benefit.”

We share this information so that members can make informed decisions about coverage and reimbursement eligibility for the services they receive. We do not share the substance of your application, including reasons an application or specific service was not approved, with members.

ACCURACY AND YOUR RESPONSIBILITIES

Carrot relies on the information you provide about your professional liability insurance and does not independently verify your coverage status on an ongoing basis. You are responsible for the accuracy of the information you submit and for promptly notifying Carrot of any change to your insurance status, including renewal, lapse, or cancellation. Carrot displays and communicates the information you provide in good faith and is not responsible for any inaccuracy in that information.

PAYMENT & APPLICATION INFORMATION

Carrot relies on Fillout to host the application form and Stripe to process application fee payments. When you complete the application, the information you provide through the form is processed through Fillout. If you pay an application fee, you enter your payment card information directly into a Stripe payment form embedded within Fillout. Stripe processes that payment information directly, and Carrot does not collect or store your full payment card number or card security code. Fillout and Stripe process personal information in accordance with their respective privacy notices:

What Carrot receives. Carrot receives limited transaction information from Stripe, such as confirmation that a payment was made, the amount and date, the payment method type and the last four digits of the card, and a transaction identifier. Carrot uses this information only to administer your application, issue receipts, maintain required business and accounting records, detect and prevent fraud, and respond to payment disputes.

YOUR PRIVACY RIGHTS AND SECURITY

Carrot’s general Privacy Notice describes the rights available to you with respect to your personal information, as well as the security measures Carrot maintains to protect it. Those provisions apply to you as a doula in the same manner as they apply to other individuals whose personal information Carrot processes, except as expressly modified by this Doula Notice.
